Request for Proposal (RFP) Instructions

The Center for Information Services (CIS) in Bellevue, Washington, provides information technology services to the thirty-four (34) Washington Community and Technical Colleges (WCTC) and the State Board for Community and Technical Colleges (SBCTC). We are deploying this RFP to assess options and to secure solutions in the area of course management.

If you intend to bid, please notify Jean Buckner by 5:00 p.m. on 11/27/07 via an email to jbuckner@cis.ctc.edu.

READ THE FOLLOWING INSTRUCTIONS BEFORE PROCEEDING. 

This RFP process is divided into 2 parts:

• PART 1 – The completion of an online survey which is due no later than 5:00 p.m. on 12/18/07.
  
  
• PART 2 – The completion of the Vendor Response Appendix (includes information which cannot be uploaded into our online survey). The appendix must be completed, saved in a Zip file and sent via email to jbuckner@cis.ctc.edu at CIS no later than 5:00 p.m. on 12/18/07.

PART 1 – The Online Survey

• Before completing the online Zoomerang questionnaire, please print the Word version of the RFP at http://www.cis.ctc.edu/pub/cmsRFP/CMSRFP.doc (which includes further instructions) and fill it out by hand. This will make it easier for you to enter your answers in the online questionnaire, as Zoomerang does not provide a Save function.
  
  
• Complete and submit the online Zoomerang questionnaire at http://www.zoomerang.com/survey.zgi?p=WEB2275MYKMR97 no later than 5:00 p.m. on 12/18/07.

PART 2 – The Vendor Response Appendix

Please complete and save the Vendor Response Appendix at http://www.cis.ctc.edu/pub/cmsRFP/VRApp.doc onto your computer. It provides a means to organize and attach additional documentation requested in the survey. The Vendor Response Appendix includes the Basic Vendor Information Form, CMS Pricing Form, Certification of Assurance, and additional required information. After completing the appendix, save it in a Zip file. Name the Zip file with your company’s name (example: XYZCompany.zip) and send it via email to Jean Buckner at jbuckner@cis.ctc.edu at CIS no later than 5:00 p.m. on 12/18/07.
